Affidavit attached: I, the undersigned, Miles Humfrey Bowker of Reedsdale, District Albany, hereby make oath and say:
1) I am a farmer residing at "Reedsdale" in the Distict of Albany and I was born at Grahamstown on the 16th August 1876.
2) At the date of my birth no system for the official registration of births in the District of Albany existed. As a result the only registration of my birth appears in the Church Registers of the Church of the Province of South Africa at Grahamstown, as will appear from the Register of Baptisms kept in the Cathedral at Grahamstown.
3) in the said Register of Baptisms my second christian name is wrongly spelt as "Humphrey" instead of "Humfrey". The name "Humfrey" is an old family name and has always been spelt "Humfrey".
4) This error was recently discovered by me and I now have taken the necessary steps to have my second christian name amended in all documents and Title Deeds registered in the Deeds Registry at Cape Town to read "Humfrey" instead of "Humphrey" My application for such amendment has been granted and all the said documents have been amended accordingly.
5) I now respectfully make application to the Very Reverend the Dean of Grahamstown, in whose custody the Register of Baptisms of the Church of the Province of South Africa at Grahamstown is kept, for the amendment of my second christian name in the said register to read "Humfrey" instead of "Humphrey".
